Dr Smedema has been working as a general and interventional cardiologistin-private-practice with the Netcare Hospital Group since 2005. He completed his PhD in 2019 and is currently completing his MPhil in Sports & Exercise Medicine. .

He is actively involved in clinical cardiovascular research of both ablebodied and paraplegic endurance athletes and is also the founder of the Sport Cardiovascular Research Program. This program is focused on sport cardiology: cardiovascular research, preparticipation screening, clinical care and original research of cardiovascular (mal) adaptations in South African athletes.
