The Faculty of Health Sciences' (FHS) Communications and Marketing Department (FHSCMD) provides branded banners and posters for use at  FHS-related events hosted by staff and student entities.

To request items, please complete the form below with your event details and return date.

Important Information:

• All items MUST be collected from and returned to the FHSCMD office (provide details of office) at the confirmed times . 

• Items must be returned in good condition and on time.

• Damages should be reported prior to or on return of items.

• The requester will be held liable for loss or damage to the items.
